The following must be completed by the Parent or Guardian of any Contestant aged under 18 years at the date of submission of the Application Form.

I hereby consent to the candidate’s participation in this election.

1. This entire entry form is for the benefit of the entrants.
2. The candidate has obtained his/her consent to participate in this election.
3. I authorize the candidate to participate in international beauty pageants designated by the conference, and, if necessary, I will agree that the candidate can leave Canada to participate in such competitions.

The Contestant is hereby informed that her personal data provided in this Application Form will be collected, retained, and used by Fairchild Television for the following purposes:

1. For contact and verification of the Contestant’s identity;
2. Arrangement of activities relating to the Contest promotion, marketing and obtaining sponsorship for the Contest; and
3. Disclosure in the Contest programs. Such programs will be broadcasted, exposed, marketed, and distributed on any media throughout the world.

I certify that I have read the Application Form for the “Miss Chinese Toronto Pageant 2026” and hereby agree to abide by all the aforesaid terms and conditions and that the information given above is true and accurate.

1. I confirm that I have never been married, have never been pregnant and have never given birth to a child.
2. I confirm that I have no criminal record in and outside of Canada.
3. I confirm that I have read and understood the content of this application form, and that if chosen to take part in the Final of the Contest, I will sign the agreement contract with the Organizer.
